Parent/Guardian Acknowledgement, Consent and Participation Declaration
Please read this declaration carefully. It is completed as part of each child’s private registration.
I confirm that I am the parent or lawful guardian of the participating child and that I have the authority to provide the acknowledgements and consents below.
I understand that participation in a school triathlon involves swimming, cycling, running, transitions, physical exertion and outdoor event conditions. These activities carry inherent risks, including slips, falls, collisions, water-related incidents, equipment failure, changing weather and accidental injury.
I confirm that the information provided during registration is accurate and complete. I will disclose any relevant medical condition, allergy, medication, support requirement or other information that may affect the child’s safe participation. Where appropriate, I will obtain medical advice before allowing the child to participate.
I confirm that the child will follow reasonable instructions provided by Tribloom, the host school, event officials, lifeguards, marshals and medical personnel.
I am responsible for providing a safe, serviceable and correctly sized bicycle and a properly fitted cycling helmet. Personal belongings and equipment should be clearly labelled and remain the responsibility of the participant and parent or guardian.
If I cannot be contacted during an emergency, I authorise trained event or medical personnel to provide reasonable first aid and arrange urgent medical assistance or transport where considered necessary for the child’s safety.
I understand that event routes, schedules or activities may be modified, postponed or cancelled because of weather, venue conditions, safety concerns or circumstances outside the organisers’ reasonable control. Any applicable refund or rescheduling will be handled according to the published Refund and Cancellation Policy.
Tribloom, the host school and event personnel will take reasonable steps to plan and operate the event safely. To the fullest extent permitted by applicable law, they will not be responsible for loss of or damage to personal belongings or for consequences arising solely from the inherent and accepted risks of participation. Nothing in this declaration excludes or limits any liability that cannot lawfully be excluded or limited.
Personal and medical information will be handled according to the Privacy Policy and used only for legitimate registration, safeguarding, safety and event-administration purposes.
